✨ Job Summary:
Seeking an experienced Occupational Therapist (OT) to provide rehabilitation services in a Skilled Nursing and Long-Term Care setting. The OT will evaluate patients, develop treatment plans, and deliver therapy to improve functional independence and quality of life.

🔹 Must-Have Skills:
✔ Active South Dakota Occupational Therapist License
✔ BLS Certification
✔ Strong patient assessment and treatment planning skills
✔ Experience in SNF / Long-Term Care settings
✔ Knowledge of ADL training and therapeutic interventions
✔ Excellent communication and teamwork skills

🔹 Experience Required:
✔ Minimum 3 years of OT experience required
✔ Skilled Nursing / Long-Term Care experience preferred

🔹 Key Responsibilities:
• Evaluate patients and develop individualized treatment plans
• Provide therapy focused on ADLs and functional independence
• Monitor patient progress and modify treatment plans as needed
• Educate patients and caregivers on therapy techniques
• Maintain accurate and timely documentation
• Collaborate with interdisciplinary rehab team
• Promote patient recovery and independence
