Essential Duties and Responsibilities:
- Determines patients' current needs to enhance daily living through telephonic call or teleportal.
- Through an individual POC assists the LCSW, Level 1 SW, Level 2 SW, Behavioral Health Case Worker and patient in identifying and meeting established goals.
- Based on identified needs, assists the patient with acquiring necessary community resources.
- Maintains an EHR of the patient's progress by documenting patient/agent interaction and progress or lack thereof.
- Develops a professional relationship with patients and caregivers to foster positive patient outcomes.
- Works effectively with Clinical Team.
- Coordinates and executes activities resulting in keeping Viemed patients "on service" with respiratory equipment.
- Maintains operations by following policies and procedures, participating in quality reviews and reporting needed changes.
- Complies with federal, state, and local legal requirements advising management on needed actions.
- Maintains client confidence and protects operations by keeping information confidential.
- Participates in reauthorization calls and assists patients with identified needs.
- Attends mandatory staff meetings.
- Attends in-services to enhance knowledge base for providing services.
- Assists patients with navigating medical options within the medical system.
- Maintains an understanding of the medical insurance process.
- Monitors 02 escalations, prevoid, and pick up reports to identify needs.
- Maintains proficiency in Therapy Notes, Bonafide, Smartsheet, Excel, One Source, Health Gorilla, and Tableau.
- Other responsibilities and projects assigned.
- Must demonstrate high ethical standards and integrity in all aspects of the role.
- Some travel is required.
Minimum Qualifications:
- Experience with medical insurance and community resources.
- Associates or Bachelor's Degree preferred.
- Provide license number if licensed.
- Confirmed State Licensed or Certification to practice or equivalent if applicable
- Valid Driver's License.
Preferred, Knowledge, Skills, and Abilities:
- Experience working in the medical field or psychiatric field.
- Medical experience with Geriatric Patients or Chronic Illness.
- Preferred- Telehealth & Telephonic Experience.
- Good listening and people skills.
